A new program called MyCareSteps has been launched in Grey and Bruce counties, Ontario, to help residents better navigate their healthcare. This initiative, developed by Serefin Health in partnership with Brightshores Health System, aims to improve care coordination for approximately 175,000 residents, especially for the estimated 23,000 individuals without a regular primary care provider. MyCareSteps offers free virtual support from nurse care coordinators who assist individuals and families in understanding their care plans, managing health conditions, and connecting with local health and community services.

This program is particularly important for people with complex or ongoing healthcare needs, as it ensures they have consistent support during transitions between different care settings. By providing a clear point of contact, MyCareSteps aims to enhance the overall care experience and help residents stay connected to their healthcare journey. The initiative reflects a growing recognition of the need for coordinated care, which can lead to better health outcomes and a more streamlined experience for patients.

Currently, MyCareSteps is in its proof-of-concept phase, having started on April 1, 2026, with plans to expand based on the outcomes and insights gained from the initial cohort of participants. The Brightshores Office of Research and Innovation is evaluating the program to assess its impact on patient experiences and care coordination. Residents can access MyCareSteps through referrals from primary care providers or hospitals, or they can self-refer to the program.
